To Order, you must first submit your recommendation documents:

Please submit copies of the following:

1. California Department of Motor Vehicles (DMV) issued driver's license, or photo ID card, issued to non-drivers. (No temporary ID's are acceptable.)
2. California Licensed Physicians Letter of Approval / Recommendation
   * NOTE: ID cards issued by other Collectives, including the Patient ID Center, are not acceptable legal forms of Physician Recommendations.

3. Mail to 420Greenhouse at 9353 Bolsa Ave. Suite D49, Westminster, CA 92683
4. We will next complete the verification of your patient status, and then you will receive an email confirming your membership to 420Greenhouse.com
5. You confirm your agreement with membership terms each time you place an order with 420Greenhouse.com.
